# Collect contact details for a specific purpose

In some cases, you may need to collect the contact details of respondents who wish to identify themselves. For example:

- Entry into a competition
- Participation in a committee or workshop
- Volunteering for an activity

Below are two options for setting up a question that allows you to collect data relating to a person’s name.

### Option 1: Multiple-choice question

The first option is 'No'. The second option is 'Yes'; please enter your name and email address below.

### Option 2: Open-ended question

You need to collect all the data on the same question.
